HACP/MCP Wellness Incentive Program

Healthy Columbus is committed to supporting you in taking proactive steps towards a healthier lifestyle. That’s why this year, we are rolling out a new opportunity for all employees in the MCP/HACP Ordinance Groups enrolled in a city-provided medical plan to earn an incentive for engaging in wellness events, programs and activities.

Who is eligible to participate?

Employees in the MCP and HACP ordinance groups (including Fire MCP, Police MCP, and Municipal Courts) that are enrolled in the Preferred Provider Organization (PPO) medical plan or High Deductible Health Plan (HDHP) are eligible to participate. 

 

Program Details
Earn a $200 incentive by completing four wellness activities beginning February 1, 2024 through October 31, 2024. Note: all awarded incentive payouts are subject to taxation.

2 | Annual Preventive Care Visit or Biometric Screening

Visit your primary care provider (PCP) for an annual preventive care visit, covered at 100%, or attend an onsite biometric screening event offered by Healthy Columbus.

On-site Biometric Screenings brought to you by OhioHealth

A biometric screening measures basic physical characteristics like your height and weight, blood pressure, blood sugar and cholesterol levels, as well as several other health indicators. These numbers provide a snapshot of your overall health and wellness. 

Knowing your "numbers" can provide information on health risk factors or indicate potential health conditions such as diabetes, heart disease or hypertension.

3 | Annual Dental Care or Eye Care Exam

Visit deltadentaloh.com and select "Find a Dentist." Scroll down to the bottom of the page and select "Search" under Delta Dental PPO and Delta Dental Premier. Under Plan Network, select "Delta Dental PPO" from the drop down list. 

Eye Care Exam

Visit eyemed.com and next to the search box, select "Find an eye doctor" in the top right corner of the page. Under Network, select "Insight Network" from the drop down menu. Type in your zip code next to "SEARCH BY ZIP" or select "USE MY LOCATION."

Do I have to enroll in the HACP/MCP Incentive Plan?

No, employees in the HACP/MCP ordinance groups that are enrolled in a city-sponsored healthcare plan are automatically eligible to participate.

How will I know when I have met all of the requirements to receive the incentive?

The Healthy Columbus team will keep track of the eligible steps and activities you have completed throughout the program calendar year. We will notify you after October 31 and indicate which activities you have completed. What counts as an annual dental exam?

A dental cleaning includes an exam by a dentist and scaling and polishing your teeth to remove any plaque and tartar build up. It is an important part of maintaining oral health. Dental cleanings should be completed every six months.

What counts as an annual physical/preventive care visit?

It is an annual visit with your health care provider to assess your current health, identify any necessary preventive care, and review health changes over time. At your annual physical, your provider will review your family and medical history, check your vital signs, and discuss lifestyle habits. Your provider will answer your health-related questions and offer guidance to improve your overall health and well-being.When scheduling your preventive care visit, ask to schedule an annual wellness care exam.

 

 

What are the differences between a preventive care visit and an office visit?

A preventive wellness visit is covered 100% by the City of Columbus if you are enrolled in a City medical plan. A preventive visit will cover your medical history and medication list. Your primary care provider (PCP) may check vitals, order screening tests, and administer vaccines and other preventive services. However, you may have to pay for services that are not preventive. Ask your provider if the services needed are preventive.

An office visit is going to collect a co-pay at check-in. The PCP can be diagnosing or monitoring a specific medical condition. You and your PCP may be addressing a specific concern or treatment plan. You may be getting a medication refill and/or requesting a referral, testing or labs for a new or addressing an existing medical condition.

Only the preventive care visit counts towards meeting the incentive requirements. 

What is the deadline for earning the annual incentive?

All eligible Healthy Columbus programs, events and activities must be completed by between February 1 and October 31, 2024. Annual preventive care visits, dental exams and eye exams completed between November 1, 2023 and February 1, 2024 will count towards meeting the incentive requirements for 2024.

 

Will I be taxed on the incentive I receive?

The incentive is subject to taxation.

Will my health information be confidential?

All Healthy Columbus programming is confidential and in compliance with the Health Insurance and Accountability Act (HIPAA). Any information shared with the Healthy Columbus team will not be disclosed, except in accordance with HIPAA Laws. 

Your Protected Health Information (PHI) will not be shared with any persons that are not HIPAA Compliance trained. Healthy Columbus will only collect and review the minimum required PHI to determine your eligibility and completion of required steps with the annual incentive plan period for the program.
